Rates
$150 per initial diagnostic intake ( this service lasts approximately 60 - 75 minutes )
$120 per 60-minute individual psychotherapy session
*Please note*: You are responsible for payment at the time of service. Unless On The Couch Psychotherapy is contracted with your insurance company or third party payer source no claims will be billed
from the practice.
Insurance
Currently, the following insurance providers are contracted with On The Couch Psychotherapy: Anthem, BCBS, Medical Mutual, & MedBen.
*Please note*: Even though your insurance company may be listed it does not necessarily mean that your services are covered. Please check your specific plan for details should you have any questions. Services may
be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
- Is there a co-pay expected during time of visit?
Reduced Fee
All clients who choose private pay or self-pay options will receive a 20% discount on all services.
Reduced fee services may be available on an individual limited basis.
Payment
Cash, debit/credit card, and personal check accepted for payment.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session.
